Along with the completed application form (Form M.9) passport applicants must supply supporting documentation including their original Republic of Cyprus birth certificate, certificate of citizenship, two recent photographs, measuring 4cm x 5cm, a marriage certificate (if you are a married woman), identification such as driving licence or identity card, birth certificates for minor children to be included on the passport and the correct fee.
Consular officials may request further proof of identity upon receipt of your passport application. It is up to you to ensure you submit all of the necessary documentation if required to do so.
Passport Applications for Minor Children
According to the High Commission of the Republic of Cyprus in London, minor children up to the age of 12 can be included in their parents’ passports or they can apply for their own passport, provided both parents give their consent. Children over the age of 12 should have their own Cyprus passport. The passport application should be submitted with all the necessary supporting documentation, but it should be signed by the child’s parents.
If you are submitting your minor child’s first-time Cyprus passport application, but you are divorced from their child’s other parent, you must still obtain the consent of the other parent or evidence of a Family Court Decision for parental care if necessary, to prove that each parent is in agreement.
All Greek-Cypriots are entitled to apply for a Republic of Cyprus passport. If you are applying for your first passport in the UK, you should submit Form M.9 along with the required supporting documentation such as your original birth certificate, marriage certificate (for married women), and other documents. Minor children up to the age of 12 are permitted to be included on their parents’ passports, but children over the age of 12 must have their own passport.
