Experienced UK drivers who want to add provisional entitlement to drive larger vehicles such as lorries, minibuses and buses will need to complete form D2 and the medical report form D4. What is involved in adding provisional entitlement to drive larger vehicles and where should applications be submitted?

DVLA has a form ordering service which makes it easier to locate all the necessary forms you need to complete and submit. Click here to get to the DVLA form ordering service page. Complete your personal details such as your name, home address, and tick the relevant boxes. Form D4 should be given to your doctor to complete. Be warned that your doctor may charge you to complete this medical report.
Form D2 and D4 are free of charge. However, you will be required to submit a passport style photograph and original documents (photocopies are not acceptable) confirming your identity such as your UK birth certificate, adoption certificate, UK passport (unless you need it to travel abroad within the next month), a national identity card issued by an EU or EEA member state, or a UK certificate of naturalisation.
If you will be submitting your birth or adoption certificate, these must be accompanied by either your National Insurance card, a letter from the Department for Work and Pensions containing your NI number, your P45, P60 or a current payslip, marriage certificate or divorce certificate or your college or university union card or school record. The reason for this is because a birth or adoption certificate is not considered absolute proof of identity.
